1) Purpose: Within the IFSP, the Transition Plan is used to document the steps and services that are taken to assure a smooth transition of the child from EI services to the subsequent activities, supports and/or service(s) that the parent(s) or legal guardian(s) choose and for which the child is eligible.

2) Process: No fewer than 90 days and no more than nine months before the child’s third birthday, the IFSP team, including the family, develops a transition plan as part of an IFSP meeting. The Transition Plan includes the steps and services to be taken to support the child and family as they transition out of the program. A Transition Plan is developed for any child transitioning from EI services, regardless of potential eligibility for preschool special education services.

3) EI Data System: This information is entered via the IFSP Home Visual Force page under the Transition tab.

  • Child's Name: Enter the child’s first, middle and last names.

  • DOB: Enter the month, day, and year (mm/dd/yyyy) of the child’s birth.

  • Date plan was started: Enter the month, day and year (mm/dd/yyyy) that transition steps and services are first added to the IFSP.

  • Transition Services: Document transition services and other activities that the IFSP team determines are necessary to support the transition of the child in this section of the Transition Plan, the Person(s) who need to be involved in implementing the service, and the Date to be completed by.

    • If a transition activity or procedure necessary to prepare the child for changes in service delivery requires provision of an EI service, document this activity and the related service within this section of the Transition Plan. For example, if a child will be attending a community toddler group to gain experience with peers in a group setting, and an EI service provider will be accompanying the parent and child, document the details of service in this section.

  • Transition Steps: Document transition steps in this section of the Transition Plan, Person(s) who need to be involved in implementing the service, and the Date to be completed by

    • Transition steps are activities that the service coordinator supports the family in carrying out and that prepare the child and family for transitioning to a new environment. These may include a discussion of documents and resources about transition or visiting potential programs.

  • Program Options: Review and document program options for the time period from the child’s third birthday forward, in this section of the transition plan. Include Person(s) who need to be involved in implementing the service, and the Date to be completed by.

    • For example, if a child turns three near the end of the typical school year, and they are eligible for preschool special education services, there may be alternate options available. Preschool and any other options to be considered when the child turns three are documented here.

  • Confirmation Checkboxes:

    • Check appropriate box:

      • AU/LEA Notified and Date of Notification, or

      • Parent opted out of notification

    • Check appropriate box:

      • Additional information transmitted to AU with parental consent, or

      • Parent did not sign consent to share additional information

    • Date Transition Conference was held: enter the date

  • Signature: Each IFSP team member signs to verify their participation in the development of the Transition Plan.

    • Transition Conference Team Member: Enter the name of each IFSP team member who participated in the development of the written plan in the appropriate row.

    • Discipline or Role: Indicate the role of the IFSP team member.

    • Representative from AU/LEA (required only for a child who is potentially eligible for preschool Part B services)
