Glossary:

Provider/Early Interventionist: Licensed and/or credentialed Early Intervention Professional

Team member: Any member of the structured team. This includes family members, service coordinators, providers/early interventionists, non-licensed EI professionals, and administrators.

EI professional: Licensed and non-licensed EI personnel.

Discipline: The area(s) of expertise based on licensure and credentials of the provider.

TEAM EI Colorado Model

The Transdisciplinary Team model selects one early interventionist as a primary provider who consistently interacts with the child and family. All Transdisciplinary team members collaborate to meet the child and family’s needs, and early interventionists must commit to teaching, learning, and working across disciplines. The person selected to be the primary provider is the best long-term match for the child and family, based on family priorities and child outcomes identified during the initial IFSP development, with the family as an integral part of the selection process.

An established transdisciplinary team of early interventionists representing multiple disciplines participates in teaming during regularly scheduled structured team meetings in person, remotely, or hybrid fashion. The primary provider receives coaching and support from all early interventionists on the team and utilizes coaching as an interactive and adult learning style to enhance the family’s capacity and confidence in promoting child learning and development based on parent-identified priorities within everyday routines and activities. The important adults in the child's life can focus on developing trust, respect, and open communication with one key person. The primary provider and family have access to all disciplines on the team, and their expertise, for ongoing collaboration, learning, and support.

Joint visits and secondary provider visits may be identified as a need through ongoing peer-to-peer coaching during structured team meetings, collaboration, knowledge sharing, role extension, and role release opportunities that naturally occur during the team meeting process.

EI Colorado Team Meeting/Teaming (Method)

A regularly scheduled structured formal meeting which is required for all early interventionists on the transdisciplinary team completed in person, remotely, or hybrid, which is a billable service. This opportunity to collaborate professionally is the beginning point for all service planning and provision, with recommendations and strategies shared freely among early interventionists to facilitate ongoing learning and coaching across disciplines and build capacity, community, and mentorship among providers and family/service coordinator participation. The family is invited to participate in the regularly scheduled team meeting time and date.

EI Colorado Coaching Interaction Style (Method)

An interactive process of observation and reflection in which the early intervention professional coaches a parent, caregiver, or other providers to support a child’s growth, development, and participation in everyday activities, experiences, and interactions with family members and peers across home and community settings.

EI Colorado Service Coordinator Definition as it relates to TEAM EI Colorado Model

The EI professional responsible for coordinating the necessary assessments, facilitating the initial IFSP meeting and subsequent reviews, describing the TEAM EI Colorado model using a primary provider, determining with the family and early interventionists the most likely primary provider, obtaining consent at the initial IFSP and any reviews, assisting the family in receiving services and supports throughout their time in early intervention described on the IFSP, and ensuring their rights and procedural safeguards.

EI Colorado Primary Provider Definition:

The one provider selected to serve as the liaison between the family and the other team members. The primary provider can be any discipline and is the best long-term match for the child and family, based on family priorities and child outcomes identified during the IFSP development, with the family as an integral part of the selection process.

EI Colorado Primary Provider Definition - Visit type

Transdisciplinary home or community-based service is delivered by one provider utilizing a coaching interaction style with the family and caregivers. The primary provider will receive ongoing support through joint home visits, and teaming discussions.

EI Colorado Joint Visit Definition - Visit Type

Transdisciplinary home or community visit in which the team identifies an additional early interventionist through the teaming process and joins the primary provider to coach, support, and guide the primary provider and family when a question or concern requires additional expertise. The joint visit must include both providers with the child and family simultaneously via in-person, telehealth, or hybrid.

EI Colorado Secondary Provider Definition:

An additional early interventionist whose expertise has been determined necessary, through the teaming process, in addition to the primary provider, to coach and support the primary provider, family, and other caregivers directly related to the IFSP outcomes.

EI Colorado Secondary Provider - Visit Type

Transdisciplinary home or community-based services are delivered by an additional early interventionist with the primary provider as frequently as possible via in-person, telehealth, or hybrid visit. As an infrequent exception, this visit type may need to be scheduled outside of the regularly scheduled primary provider visit. If this occurs, ongoing collaboration and communication between both providers and team discussions is necessary to support the family with this type of visit.

The workgroup came to a consensus to discontinue the use of consult as an EI CO definition, as primary provider visit, joint visit, and secondary provider visit types meet the needs of TEAM EI CO model.
