Article summary

Purpose: The “Status” field is used to capture the process of the EI Referral Case from initial referral through eligibility determination.

  1. Status Field

    1. New - This is the default “Status.” New indicates the referral has been received but no action has been taken on the part of the Service Coordinator/Care Navigator or CCB

    2. In Progress - In Progress status indicates that the referral is being worked on and is assigned to a Service Coordinator/Care Navigator to initiate contact, schedule evaluation, gather information, etc.

    3. Closed - Closed status indicates the EI Referral is being closed either because the child is not eligible for Early Intervention or for another Closed/Not Enrolled reason

      1. Closed Cases require a “Final Referral Outcome.” The options are as follows:

        1. Deceased

        1. Declined Participation

        2. Duplicate Referral

        3. Incomplete Referral

        4. Parent declined - not sure why referred

        5. Wrong Service Area

        6. w/in 45 days or over 3

        7. Attempts to Contact Unsuccessful

          Attempts to Contact options:

            1. Missing Phone #

            2. Disconnected

            3. Wrong Phone #

            4. Passive Decliner

            5. No Answer - Passive Decliner

        8. Transferred Case

    4. Complete - Complete Status indicates the child has been found eligible for Early Intervention and will be transferred to the CCB for IFSP development

      1. “Final Referral Outcome” field must be “Client Case Opened” - this is the only option available

        NOTE: When an EI Referral Case is saved with a Complete status, the EI Data System will automatically generate the EI Case

  1. Date of Closure - The date the EI Referral Case is saved in either a Closed or Complete status

  2. Case Closure Notes - This is an optional field to add any notes related to closing the EI Referral Case
