Setup Existing Provider as Evaluator
Purpose: How to register as an evaluator when already a Provider with a Portal login
Navigate to My Personal Information tab. Under Provider Information/Contact and Personal Information, there is a checkbox for Are you also an Evaluator
Check the box and Save
Now there will be additional tabs available under the MORE tab for EVALUATOR INFORMATION
