Service Provider Assignment - Service Plan Element

Purpose: Service Coordinators must use the Service Provider Assignment function to assigned a service provider to the Service Plan Element.

1. Navigate to the Service Provider assignment: Client Case>IFSP>Service Plan Element> Service Provider Assignment

2. From IFSP, click on Related tab and scroll down to the Service Plan Elements section

3. Locate the correct Service Plan Element and click on the link

4. Click on the blue Service Provider Assignment button in lower right corner

5. Search and Select the Service Provider

6. Click in the Start Date field and select a date from the calendar pop up

7. Search and Select Service Provider contact (the Service Provider Assignment will populate automatically with the date ranges of the Service Plan Element upon Save)

8. If an End Date is not entered, the field will populate with the IFSP end date of the associated Service Plan Element upon Save

9. Initial IFSP Case and Screening Client Case should remain blank

10. Service Plan Element field automatically populates

11. Click Save

12. The Service Provider field on the Service Plan Element will update to the Service Provider assigned to the most recent Service Provider Assignment.

13. All Service Provider Assignments can be viewed on Service Plan Element while clicking on the Related tab

14. If an additional Service Provider Assignment is created, the original Service Provider Assignment End Date will be the day prior to the New Service Provider's Start Date
