Purpose: Service Coordinators must use the Service Provider Assignment function for a Service Provider to bill via the Provider Portal for services provided to a child.

Assigning Provider for Initial IFSP

1. Client Case → Related → Service Provider Assignments (Initial IFSP Case) → New

1. Select Provider that will participate in Initial IFSP development, Start Date and End Date will be left blank

Setting Up Services in the EI Data System

1. IFSP → Service Plan Element → Related Tab → Service Provider Assignment → New

• 1. Or Service Provider Assignment button from IFSP → Service Provider Assignment

1. Create New Service Provider Assignment

   1. Enter Service Provider

NOTE: If Late Service Received is checked a Late Reason will need to be entered prior to creating New Service Provider Assignment

Changing the Service Provider

1. From the Service Plan Element select Service Provider Assignment button

2. Select a new Service Provider

   1. Enter Start Date

   2. The prior service provider will be end dated
