Purpose: The Prior Service section of the EI Referral Case is to document a child’s prior involvement with therapy services, either from Early Intervention Colorado, clinic or home health services or services from another state.

1. Did the Child previously receive service - This is a checkbox. When checked, additional fields become available to document details about prior services received before the referral to Early Intervention.

2. If yes, where? - This field has three options:

   1. EI

   2. Private Therapy

   3. Out of State

3. Prior EI Case # - This field allows the users to link the child’s prior EI Case in the EI Data System to the Referral Case for quick reference.

4. Details about Prior Services - This field is a narrative box where high-level information about prior services can be saved for quick reference for the Service Coordinator/Care Navigator

   1. NOTE: Details about prior services should also be documented in case notes. See Tasks & Events.
