Article summary

Purpose: To address the need for documenting out of state transfers within the EI Data System. Children who transfer from another state and meet Colorado’s eligibility criteria for a significant delay will be documented as eligible due to an established condition “Out of State Transfer”. This will ensure the child’s initial IFSP captures eligibility while also reflecting current developmental functioning within the five developmental domains.

  1. Review the original IFSP from the  prior state and determine whether evaluation results meet Colorado’s eligibility criteria

    1. If evaluation results do not meet eligibility criteria → offer family an evaluation

    2. If evaluation results do meet eligibility criteria → proceed to creating Initial IFSP in Colorado

  2. Enter “Out of State Transfer” in the Primary Diagnosis field on the EI Referral Case

  3. Create the EI Screening Record

    1. Eligibility must be documented on the EI Screening Record in order to copy to Initial IFSP

    2. Child will be eligible due to an established condition “Out of State Transfer”

    3. Hospital/Practice = Out of State Transfer

    4. Qualified Health Professional = Name of original SC and state

    5. Credentials = Physician Assistant (will change to Parent Report when Credential list is updated)

  4. Date of Evaluation and Date of Eligibility Determination will be the date of initial Assessment

    1. Developmental Domain information will reflect current levels of development and range of delay(s)

  5. Create Initial IFSP  

NOTE: If child is eligible due to an established condition reflect the child’s accurate diagnosis in the EI Data System
