Letter of Request - Waiver Process

Purpose: This document outlines information required in the Letter of Request for waivers.

• Standard Waiver:

  • Letter of Request: Detailed statement completed by the Local EI program or agency for why the standard waiver is needed. This should include specifics for supervision including:

    • Name and qualifications of supervising professional

    • Reference adherence of the Supervisor of Providers with Standard Waivers

    • Plan for direct supervision

    • Supervisor attendance at IFSP meetings with the acknowledgement that the supervisor is the one to develop, review, and/or modify IFSP outcomes and strategies

• Specialized Developmental Intervention Waiver:

  • Letter of Request: Detailed statement completed by the Local EI program or agency for why the standard waiver is needed.

• Developmental Intervention - Assistant Waiver:

  • Letter of Request: Detailed statement completed by the Local EI program or agency for why the standard waiver is needed. This should include the specifics of the plan for supervision including:

    • Name and qualifications of supervising professional

    • Reference adherence to the Competencies for Developmental Intervention-Assistants and receive supervision by a provider that meets EI Colorado Personnel Standards and provides supervision in accordance with the Competencies for Supervisors of Developmental Intervention-Assistants

    • Plan for direct supervision

    • Supervisor attendance at IFSP meetings with the acknowledgement that the supervisor is the one to develop, review, and/or modify IFSP outcomes and strategies
