Individual Provider: Adding Insurance Information

Specify the types of Insurance Billed by the Service Provider

1. Select Insurance Information by clicking the Available Insurance Types, and then click the ► symbol to add the Insurance Type to Selected. Select all that apply

2. Multiple Insurance Types may be selected at once by holding down the ‘CTRL’ key and clicking multiple available insurance types.

3. Click the ► symbol to move from Available to Selected

Enter the Medicaid Provider ID if the Service Provider is Medicaid Enrolled. The Medicaid Provider ID is REQUIRED if the following services are provided:

1. Speech Language Pathology

2. Occupational Therapy

3. Physical Therapy

4. If the Service Provider does not yet have a Medicaid ID and provides Speech Language Pathology, Occupational Therapy, or Physical Therapy services, the Provider can indicate an allowable reason for not yet having a Medicaid ID.

5. Select one of the four options in the dropdown

   1. Pending Medicaid Enrollment

   2. Physical Therapy Assistant

   3. Occupational Therapy Assistant

   4. Speech Language Pathology Assistant
