Individual Provider: Adding Information and Certificates/Documentation

  1. Enter Years of Experience with Children Ages 0 to 3:    

  2. The Waiver Section is only to be filled in for those who are required to have a waiver in place. Clicking on the Request Waiver button will bring up a new page with options to fill out. You can access the user guide for this section here.

  3. In Discipline/Certificate section click on Add Certificate

    1. Choose the discipline from the drop down

    2. Enter the license or certification title

    3. Enter an expiration date, if applicable

      1. Note: Some disciplines will require an expiration date to be entered prior to saving

      2. Note: If a license or certificate has expired the provider’s overall status will be Expired and they will not be able to provide services.  If a license or certificate is about to expire the provider will be notified 30 days in advance to update

    4. Check the Waiver box if waiting for documentation of degree or licensure

    5. Click Save or Save & New

  4. Once a user saves the discipline, they have the option to upload any certifications

    1. Click on ▼ symbol  and Select Upload

    2. Click on Upload Files Choose a file to upload.  

      1. Note: Allowable document formats are .pdf and .png

      2. Note: The Upload Files area may appear below the section so you may need to scroll down to find it.

    3. Click Done

    4. Add as many disciplines as applicable

  5. Insurance can be added and uploaded in the Insurance section by clicking the Add Insurance button

  6. Training can be added and uploaded in the Insurance section by clicking the Add Training button

    1. Note: If desired training not listed, training can be entered under Discipline/Certificate section by typing in the training name under License/Certificate Title field

  7. Select Service Model

    1. Select a Service Model by clicking the Available Service Model and then clicking the ► symbol to add the service model to Selected. Select all that apply

  8. Select Specialty Areas (Optional)

    1. Select a Specialty Area by clicking the Available Specialty Areas and then clicking the ► symbol to add the Specialty Area to Selected.

    2. Multiple Specialty Areas may be selected at once by holding down the ‘CTRL’ key and clicking multiple available specialty areas.

    3. Click ► symbol to move from Available to Selected

  9. Select Provider Services

    1. Note: Provider Services are services that are entered on a child’s IFSP

    2. Select Provider Services to be provided

    3. Highlight Service Model.

    4. Multiple Provider Services may be selected at once by holding down the ‘CTRL’ key and clicking multiple services.

    5. Click ► symbol to move from Available to Selected

  10. To update any existing documents

    1. Click on the ▼ symbol  to the right of the document name and select Edit

    2. Put in the expiration date for the new document and click Save

    3. Click on ▼ symbol and click Upload

    4. Click on Upload Files and select the new document and click Done

      1. Note: You may have to scroll down the page to locate the Upload Files button
