Independent Subcontracted Provider

Provider submits an EIB request. The request has a provider name listed, and the Hospital Practice Field will either be blank or say EIColorado Provider Community Account. If there is a company listed you will need to verify with the provider if they are with a provider agency with multiple providers or if they are the only provider in that company. If they are the only provider proceed with these instructions. If there are multiple providers in the company,y see the instructions for providers within a company below.
The email listed in the Account for the EIB in the CCB Request Email field will receive an email letting them know that the provider wants to work with them, including a link to the EI CCB Staff Object for that provider.
EIB staff will review the documentation for the provider found in the Requests section of the provider contact record (on the related tab) and then update the EI CCB Staffs record with the subcontractor checkbox checked and the Status updated to Verified. Add the hourly rate. Then click save. This will update the Attest Status automatically to pending provider signature. Notify the provider that the attestation is ready for them to sign.
Provider logs in to the provider portal and signs the attestation form. They will need to let EIB staff know once they have done this, as there is no email notification.
EIB staff will then reopen the EI CCB Staff object (this can be found on the contact record for the provider in the related tab and EI CCB Staff section). Sign the CCB Attestation by using the button in the upper left-hand corner of the screen. Once the form has been signed and submitted, staff can refresh the page, and the Status will update to Active and Attest Status to Complete. The provider is now an active subcontracted provider.
