1) Purpose: The IFSP Outcomes/Plan of Action page is used to document a meaningful outcome for the family and the child that is functional and measurable, and to identify strategies that will address the outcome(s).

2) Process: The entire IFSP team participates in completing this step of the IFSP process. During this step of the IFSP process, the team refers back to pertinent information gathered earlier in the IFSP process.

3) EI Data System: This information is entered via the IFSP Home Visual Force page under the IFSP Outcomes/Plan of Action tab.

1. What is the priority this outcome will address?

  • Write the priority exactly as it is stated on the Concerns and Priorities page.

Complete the IFSP Outcome section as follows:

2. IFSP Outcome: What would you like to see happen?

  • Describe the change the family would like to see in a daily routine or activity, including the desired behavior of the child.

3. Measurable Criteria: Where are you starting?

  • Describe, using jargon-free language, what the family and other IFSP members see now that reflect the child’s developmental skills as related to the outcome.

4. Measurable Criteria: How will you know when the IFSP Outcome is achieved?

  • Describe how the routine will be different when the IFSP is reviewed (minimum of six months, but should be based on a family-identified timeline).

5. Strategies Activities and Learning Opportunities: What steps need to be taken to help accomplish the IFSP Outcome?

  • To complete this section the IFSP team, including the family, brainstorms all of the strategies that should be considered for addressing the outcome within the child/family’s everyday routines, activities, and places. The team then chooses the strategies that best address the outcome.

Global Outcomes Addressed:

  • Document the Global Outcome that the IFSP outcome aligns with.

  • Family Outcomes: A family may prioritize a concern that is not directly related to the child. In this case, a “family outcome” will be developed. Family outcomes may require assistance from the service coordinator to access resources, but do not require an EI Service. The intent of family outcomes is to support the family to accomplish the outcome.
