Forms

Purpose: Forms are available via the EI Data System on the EI Client Case. When a form is launched, fields will auto-populate with information from the EI Data System. All populated data is editable if needed. When launched, each form can be translated into Spanish and/or Saved. If the form is Saved, it will be part of the child’s EI Record in EI DATA SYSTEM. The form can be printed and used to gather a wet signature.

Launch form by clicking on the link of the desired form located on the right side of the screen within Client Case Details screen layout

1. The following forms are available for selection:

   1. AU Notification

   2. Consent to Evaluate

   3. Prior Written Notice

   4. Health Insurance Authorization

   5. Telehealth Recording Statement

2. To generate an English PDF of the form:

   1. Enter all text and date fields

   2. Click Save

   3. Click Print PDF

3. To generate a Spanish PDF of the form:

   1. Enter all text and date fields

   2. Click the Translate icon

   3. Click Save

   4. Click Print PDF

4. All saved forms can be accessed from the Client Case by

   1. Clicking on the Related tab

   2. Scrolling down to the EI Forms section at the bottom of the screen

5. To generate a Print PDF from a saved form located in EI Forms

   1. Click on the unique alphanumeric name under EI Forms Name (e.g. a4H7b000000ecb0)

   2. Click Print PDF in the upper right corner

   3. Select the appropriate language from the following options:

      1. English

      2. Spanish

   4. And click Print
