  • EI Colorado will email an introduction letter to families at 2 years 3 months

  • EI Colorado will email a survey to families to be completed at one of the following times:

    • 2 years 6 months, 2 years 9 months, OR when your child exits the program

What is the Family Outcomes Survey?

  • EI Colorado utilizes the Family Outcomes Survey-Revised (FOS-R 2010): Part C, developed by the Early Childhood Technical Assistance (ECTA) Center which consists of two parts:

    • Section A focuses on the 5 family outcomes and contains 24 items

    • Section B focuses on the 3 helpfulness indicators required for Office of Special Education Programs (OSEP) reporting and contains 17 items

  • These data are required by the Individuals with Disabilities Education Act (IDEA) and are included in annual reporting to OSEP for local programs and the state

  • Families generally take about 10 minutes to complete the survey

How can service coordinators and providers support the family in completing the Family Outcomes Survey?

  • The service coordinator (SC) will explain the purpose and importance of the survey during transition planning for the child. The SC will document this discussion in the transition plan of the EI Colorado Data System

  • SCs and providers can help administer the survey to families that do not have an email address or internet access through a link that will be provided to Local Early Intervention Brokers

  • The SC will determine the language needs of the family prior to 2 years 3 months old and contact Enid Torres at [email protected] to obtain the appropriate FOS-R

    • The FOS-R is currently available in 17 languages
