Timeline and Process

2 years 3 months

  • Introduction Letter is emailed to families by EI Colorado

    • What is the Introduction Letter? The Family Outcomes Survey (FOS) introduction letter informs families about the survey and reminds them of the collaboration and connection between Early Intervention (EI) brokers and EI Colorado. The purpose of this letter is to provide clarification about the survey and why they are receiving it

  • The service coordinator informs the family of FOS during monthly contact

    • Note: If the family does not have an email address, it is the Service Coordinator’s responsibility to share the introduction letter

  • SC will inform the family about the FOS and document this in a case note. Additionally, during transition plan development, the SC will provide information about the FOS to the family and document it on the transition plan

2 years 6 months

  • Families receive survey

    • Families with an email address will receive the link via email

    • Families who do not have email addresses will receive a link from their service coordinator through text message or can request to speak to the Bilingual Quality Assurance Coordinator to complete survey via phone

    • Note: If the family completes the survey at 2 years 6 months, they will not receive the survey again

2 years 9 months

  • Families receive survey

    • Families with an email address will receive the link via email

    • Families who do not have email addresses will receive a link from their service coordinator

    • Note: If the family completes the survey at 2 years 9 months, they will not receive the survey again

3 years old

  • Families receive a survey when their child turns 3 years old and exits the program

    • Families with an email address will receive the link via email

    • Families who do not have email addresses will receive a link from their service coordinator

    • Note: If a child is participating in the Extended Part C Option, they will receive the survey at the same points listed above
