Purpose: This tab collects additional demographic information for the evaluator so that evaluation slots can be matched to children requiring an evaluation

  1. Evaluator Discipline - This field captures the areas of development an evaluator can assess based on their license. Only the Company Owner can update the Evaluator Discipline. This will be matched to the child’s primary area of concern. A provider evaluating in any discipline(s) selected must meet the Qualified Personnel Standards for the discipline selected. Available options:

    1. Speech

    2. Physical

    3. Adaptive

    4. Cognitive

    5. Social Emotional

  2. Evaluator Language - This field captures the language(s) an evaluator is fluent in and can complete an evaluation without the need for interpretation. Available Options:

    1. English

    2. Spanish

    3. Amharic

    4. Arabic

    5. ASL

    6. Burmese

    7. Cantonese

    8. Chinese (Mandarin)

    9. Croatian

    10. French

    11. Hindi

    12. Hmong

    13. Italian

    14. Japanese

    15. Khmer

    16. Korean

    17. Lao

    18. Malay

    19. Mongolian

    20. Nepali

    21. Portuguese

    22. Russian

    23. Somali

  3. Evaluator County - This field captures the counties in which an evaluator is willing to complete evaluations. Evaluations can be completed virtually, in person or a hybrid model. Available Options:

    1. All Colorado Counties

  4. Evaluator Zip Code - This field captures the zip codes within each selected county the evaluator is willing to complete evaluations. The zip codes displayed are dependent on the county(ies) selection. Ex. When Washington County is selected, only Washington County zip codes are available for selection.

  5. When all evaluation information has been entered click Save on the bottom of the screen
