IFSP Plan of Action - IFSP Home

Article summary

  1. While in the IFSP Home

  2. Click on the IFSP OUTCOMES/PLAN OF ACTION tab

  3. To enter a new Plan of Action, click the button in upper right corner + Add Plan of Action

  4. Enter text in corresponding boxes to answer the following:

  5. What is the priority this outcome will address?

    1. IFSP Outcome: What you would like to see happen?

    2. Measurable Criteria: Where are you starting?

    3. Measurable Criteria: How will you know when the IFSP Outcome Is achieved?

    4. Strategies: What steps need to be taken to help accomplish the IFSP Outcome?

  6. Select the correct Global Outcome Addressed from the following drop-down menu:

    1. Taking Appropriate Action to Meet Needs

    2. Acquiring and Using Knowledge and Skills

    3. Developing Positive S/E Skills

    4. Not Applicable/Family Outcome

  7. Click Save

  8. If creating a review and entering a new outcome, New Outcome will automatically be selected under Outcome Status.

  9. If reviewing a previously established Plan of Action, use the text boxes corresponding to Outcome, Progress, and If Outcome not met, please explain.

  10. Select correct Outcome Status:

    1. Outcome Met

    2. Not Met: Continued

    3. Not Met: Revised

    4. Not Met: Discontinued

Note: When navigating to this tab within the IFSP Home Visual Force during a Periodic or Annual Review, the system will mark previously established IFSP Outcomes with Outcome Status Pending Review IF the outcome was previously New Outcome. All other Outcome Statuses will remain the same as the previous IFSP.

  1. IFSP Meeting Date and IFSP Type displays in the blue Plan of Action bar above each outcome

  2. Select the Include on Print? checkbox for each Outcome that should be included in the IFSP Print PDF
