Extended Part C - Data System Information

Purpose: The Extended Part C Option allows a subset of children to continue Part C Services past their third birthday until the first day of the school year following the child’s third birthday.
All Indicator 8 (Part C Transition) timelines and requirements DO NOT Change
The Extended Part C Option will be a part of Transition Planning (if applicable)
Administrative Unit (AU) Notification and Timelines remain the same
Transition Conference with AU Representative will include an explanation of the timing of Part C Extended Option
Indicator 3 timelines DO NOT change
Global Outcomes timelines remain the same
Exit rating completed within 90 days before the child turns 3
A new checkbox is available on the EI Case screen “Part B Eligible - Extended Part C”
This is to be checked for any child aged 3 or older participating in Extended Part C
This box cannot be unchecked if more than 2 weeks have elapsed since the child’s third birthday
Status to remain Active
Service Coordinator must update the end date(s) for each service on the IFSP
This should be done as part of a review meeting
Periodic review if the timeline will last throughout the extended time period
Annual review if an annual would be needed after the child turns 3 (move the date of the annual review up to an earlier date)
Service Coordinator must update the end date(s) for each Service Provider Assignment
Service Provider Assignment is on the Related tab of the Service Plan Element
When the child exits the Extended Part C Option there is a new Closed/Eligible Exit Reason
“Part B Eligible - Participated in Extended Part C” and enter the ID on the Contact
