EI Screening Record_Eligibility Information

Purpose: The EI Screening Record is now available on the EI Referral Case to capture eligibility determination. By completing the EI Screening Record, the Eligibility Information section of the Referral Case is updated.

1. Note: By July 1, 2022, the Evaluators will have access to the EI Screening Record via the Provider Portal. It will be the expectation of the evaluation team to complete the EI Screening.    

2. To access the Early Intervention Screening record, the Care Navigator will click the Early Intervention Screening box in the upper right corner of the EI Referral Case. This will launch the Evaluation Lightning Home visual force page. The following section on the EI Screening can now be completed:

   2. Child Evaluation or Assessment & Medical Information

      1. Nutrition and Growth

      2. Dental

      3. Vision

      4. Hearing

   3. Methods and Procedures Used

      1. Method

      2. Tools

   4. Developmental Domains

      1. Range of Delay

      2. Developmental Level Min Age and Max Age

      3. Results Summary Including Qualitative Information

   5. Eligibility

   6. Evaluation Team Participants

      1. Role

      2. Discipline

         1. Note: Only existing Contacts can be entered  

3. Completion of the EI Screening will update the Eligibility Information section on the EI Referral Case

4. The EI Screening Record and Eligibility Information will transfer to the EI Case when created
