Article summary

Purpose: When the ongoing Service Coordinator receives an eligible child, and the EI Screening Record has not been signed by the parent/guardian (has not been obtained by a Care Navigator)

  1. Care Navigator is responsible for sending the EI Screening Record to DocuSign Recipient #1 for signature

  2. Care Navigator updates EI Referral Case Status to “Complete” and the new EI Client Case is created for EIB

    1. If evaluation results come back with a signature before the EI Client Case is created, the EI Screening Record (EI_Initial_EvalAssessment_EN.pdf) and Certificate of Completion will be attached to the EI Client Case → Notes and Attachments section

    2. If evaluation results come back after the EI Client Case is created the EI Screening Record (EI_Initial_EvalAssessment_EN.pdf) and Certificate of Completion will only be attached to the EI Referral Case → Files. It will not be saved to the Client Case. The Service Coordinator can make a copy from the EI Referral Case and save in the Notes and Attachments section. The Owner of the EI Referral Case will receive the “Evaluation Signed” Task.

      • If the EIB receives an eligible child and DocuSign signatures still need to be received, the ongoing Service Coordinator will prompt families to search for an email from Care Navigator to locate EI Screening Record to obtain a signature. DocuSign sends email reminders every two days
