    1. In the EI CCB Staffs object for that provider, update the Status to No Longer A Provider and enter an end date. DO NOT uncheck the Sub-Contractor checkbox.

    NOTE: The end date is critical for billing purposes and must be accurate.

    1. Click save.

    1. The provider will submit a new CCB Request

    2. Go through the process to set up a provider as an employee
