Eligibility Information

Purpose: Eligibility is determined during a multidisciplinary evaluation or based on the confirmation of an Early Intervention Colorado approved diagnosed Established Condition that is associated with a high probability of resulting in significant delays in development. Information in this section of the client case will be populated based on information entered on the Evaluation Screening record or within the Health Information/Established Condition section of the EI Referral Case. Instructions for completing an evaluation and entering a screening record can be found in the EI Screening/Evaluation Results article or Entering EI Screening Evaluation Record article.
