DocuSign - Periodic Review (PR) Transition Conference

Purpose: If the Transition Conference occurs outside of an IFSP meeting, the Service Coordinator can send only the Transition Conference details via DocuSign and obtain only the signatures relevant to the Transition Conference. When the Transition Conference occurs in conjunction with an IFSP, all individuals listed in the Transition Conference Team Members table will be included in the IFSP DocuSign signing order; there is no need to create a standalone PR-Transition Conference.

• Create Review from Client Case

• Select PR-Transition Conference

• Click Create

• New PR-Transition Conference IFSP will be created

  • Only the Background Information and Transition Tabs are available

• Enter/Add information to the Transition tab

• Add Team Members to the Transition Conference Team Members table

• Save

• Check the checkbox for Include on Print

• Select Print PR

  • DocuSign window pop-up for saving as a DocuSign draft

  • Select OK

• Send PR-Transition (same functionality as Send with DocuSign button)

• PR-Transition will send to all individuals listed in the Transition Conference Team Members table

NOTE: Ensure the AU Representative/Child Find Contact email is entered and accurate.  If any emails are missing or invalid, the envelope will fail to send.
