Purpose: The Department of Human Services Involvement section of the EI Referral Case is to document any potential involvement in DHS and the relevant Contacts for the child.
Involved with the Dept of Human Services -Yes/no. If no, the section collapses so that fields not in use are no longer visible on the EI Referral Case layout. If yes, additional fields are made available for the Service Coordinator/Care Navigator to complete.
County Office - Dropdown of all Colorado counties. This will indicate the DHS county office for the child.
Caseworker Name, Caseworker Email, Caseworker Phone - These fields are free text and can be used to capture the contact information of the assigned DHS Caseworker
NOTE: A contact is not created in the EI Data System
GAL Name, GAL Email, GAL Phone - These fields are free text and can be used to capture the contact information of the assigned GAL

Person with whom the child is living? - Free text field. Use clear, concise language so that all relevant parties can understand the child’s living situation.
Were parental rights terminated by the court - Checkbox. When the box is checked, it indicates that parental rights have been terminated by the court.
Legal Status of the Child - Document the status of parental rights
Biological parent custody, rights intact
Foster/kinship, biological rights intact
Foster/kinship care, parental rights terminated
CAPTA Referral Source - If yes, CATPA Referral must indicate the following:
Child welfare/CAPTA - This would be a referral from TRAILS or a DHS representative that indicates at the time of referral that CAPTA applies.
EI State Office/CAPTA - This is an instance in which EI Colorado determines the CAPTA referral was never sent to the CCB/Central Intake although CAPTA applies.
