Child Find/Transition Information - Client Case

Article summary

Purpose: The Child Find/Transition Information section of an EI Client Case contains the following fields:

  1. School District - Look Up

  2. Child Find Contact - Look Up - If a Child Find contact does not appear users will need to verify that the contact has been added.  To do this, users can do a Global Search on the contact.  If they do not locate a contact record, a new contact will need to be create

  3. 2 year 3 months – Calendar

  4. 2 years 6 months – Displays the Date when the child’s age will be/is 2 years 6 months old

  5. Transition Plan Date – Updated based on the IFSP

  6. Additional Information Shared – Text field

  7. Date Transition Conference was Held – Calendar

  8. Client LEA Notification Email - Text box

  9. LEA Notification opt-out? - Pick list, Yes or No.  This field is populated on Indicator 8B

  10. 2 years 9 months – Calendar

  11. AU Notification

  12. AU Date Sent
