Case Information

Purpose: The Case Information section of the Client Case contains the following fields:

1. Case Age (Days) – Calculated automatically based on the date the case was created. This field is available only after the page has been saved.  

2. Projected Initial IFSP Date - Populated based on Initial Referral date plus 45 days

3. Projected 6-month review - Populated based on Initial IFSP Meeting Date plus 183 days (approximately six months)

4. Projected Annual review - Populated based on 364 days after Initial IFSP Meeting Date

5. Last annual review - Calendar

6. Projected Transition Plan - Populated based on child’s date of birth plus 1005 days

7. Last periodic review - Calendar

8. Medicaid Enrolled – Pick list - If a user selects Yes and the Medicaid ID is not entered on the Client Contact, users will be prompted to go back and enter the ID on the Contact.
