Purpose: The evaluation team bills for Evaluation, Interpretation and/or No-show via the Provider Portal

1. From the My Client Cases for Evaluation tab

2. Click the hyperlink to Add Service

3. A new window will open to Create Activity

   1. Enter the Date - this will be the date of the evaluation/no-show

   2. Select Activity Types

      1. EI: Initial Evaluation

      2. EI: Eval No-show

         1. Progress note must be attached to the child’s case for each billed no-show activity

      3. EI: Eval Interpretation (for no-shows & late cancellations as well)

   3. Units/Miles - enter units in 15 minute increments

   4. Rate Hourly (does not display for EI:Initial Evaluation as the flat rate is $527.88)

   5. Activity Description (Optional)

Note: Activities cannot be created for future dates.
