After Evaluation Process

After the evaluation, it is the responsibility of the evaluators to share eligibility information with the family.
If possible, this information can be shared immediately following the evaluation
If it is not shared then, it should be shared with the family within 24 hours of the evaluation
On the EI Screening Record it should be noted who shared the results with the family:
Enter the Primary and Secondary Evaluators in Evaluation Team Participants
Other Evaluation participants will be documented in Evaluation Comments
Consent Statement should document how the evaluator participated in the evaluation
Ex. Participated via Zoom
Signature of Evaluation Participant will not be collected. Signature line will read “intentionally left blank”
Signature of the EI Representative will be blank. Signature line will read “intentionally left blank”
Consent Statement field in the Eligibility section - in this field evaluators note who explained eligibility, and to whom. (there is a 225 character limit)
Name of evaluator, discipline, explained evaluation results and eligibility determination to, Name of parent, relationship to child.
Ex. Laura Bernard, OT, explained evaluation results and eligibility determination to Amy Smith, mother.
NOTE: Out of home placement: Name of evaluator, discipline, explained evaluation results and eligibility determination to foster or kinship parent. (Do NOT note the name of the foster parent.)
Guidance for quantitative information when child is in out of home placement: Do not use names other then the child’s, if information is provided by caregiver note as “per foster parent/kinship parent”
Evaluator will click “Submit and Complete” on the EI Screening Record
Entering Evaluation Results
On the referral case in the Provider Portal, then “Launch EI Screening'' to enter the evaluation.
Evaluators will input evaluation results, including eligibility determination, into the EI Screening Record in the Provider Portal within 72 hours of the evaluation
In each area of development evaluators should note which evaluator completed that area of development - this can be full name, or initials
In addition to the range of percent delay and the min/max age, note the detailed percent delay in the qualitative information box for each area of development
On the referral case in the Provider Portal, then “Launch EI Screening'' to enter the evaluation.
Interpreter present: Evaluators please note on the “additional information” field if an interpreter was present
