Scope of Work Review and Intake Questionnaire Demonstration

E1-4: Conduct intake activities utilizing the EI Colorado Care Navigator Script and Process document. Families shall have the same experience regardless of whether they have an EI Colorado Care Navigator or a local Care Navigator. / The online intake questionnaire shall be used for every referral.

  • Care Navigator Script and Process Guide

    • EI CO will be making updates to this document

  • The intake questionnaire should be filled out in English in the Data System, even if filling out the Spanish intake questionnaire

  • Question: Many families will say their child is 35 and 4/7th weeks or something similar. What should they enter if a family gives a number like that?

    • Rounding up or down is currently required because that field in the data system requires a whole number. Best practice is to use an associated free text field to document actual weeks and days when needed, such as 'What was your child’s birth weight?' on the intake questionnaire.

    • EI CO can follow up with the data team to see about entering decimals, weeks, and days, etc.  

    • There are suggestions to round down, as this question informs evaluators and providers about prematurity.

  • Required for all cases except for established condition cases because children with established conditions undergo initial assessment (not initial evaluation)

Evaluation Late Reasons

  • Family versus system late reasons

  • The late reason selected should be supported with case notes in the Data System

  • 21-day target is not a compliance indicator

Provide Examples of Late Reason Scenarios

  • Walked through choosing a late reason and documenting it in a case note

  • If the evaluation is scheduled after the 45-day timeline, referral and intake staff decide if the evaluation was late because of a family reason or a system reason. Late reasons paint a picture for EI Colorado to understand how the EI system is operating statewide and if compliance indicators are being met. Entering the correct late reason helps us understand what is going on statewide. Referral and intake staff should use their best judgment to select the correct late reason. If you need help with selecting a late reason, please reach out to EI Colorado or bring your questions to office hours.

    • DDRC asked if a family or system late reason was more appropriate for a case that she is currently working on. Office hours participants discussed the case together and determined the correct late reason to use.
