EI Screening Record/Evaluation Results
DocuSign
Yes. Evaluators have been removed from the signing order on the EI Screening Record. Parent signature is the only requirement on the EI Screening record acknowledging that the parent participated in the evaluation, the results were shared, an eligible determination was made, parent understands their Rights and Procedural Safeguards. A parent is not required to provide consent at this time.
The Care Navigator will send the EI Screening Record via DocuSign for parent signature.
No. There are a number of EI Screening records that have not been sent via DocuSign as they are in a Waiting Status. Upon removing the evaluators from the signed order all Waiting EI Screening Records will be sent to the parent (DocuSign Recipient #1) for signature

Yes. Evaluators will no longer “Send with Docusign” via the Provider Portal. Care Navigators will send the EI Screening Record to the parent for signature. Evaluator signatures are not required on the evaluation results/EI Screening Record

Eligibility Consent Statement
Yes. In either the eligible or not eligible section of the EI Screening Record a new field for “Consent Statement” has been added. The evaluator should indicate the following information:
Name of evaluator, discipline, explained evaluation results and eligibility determination to, Name of parent, relationship to child.
Ex. Laura Bernard, OT, explained evaluation results and eligibility determination to Amy Smith, mother.
