Creating an Online EI Referral Case

Purpose: A public facing online Early Intervention Referral Form is now available on the EI Colorado website (A Spanish version of the referral form can be accessed by clicking on the “Enlace al formulario en español” link. Anyone in the community can submit an online referral to EI. Upon completion and submission of this form, an automated referral is created in the UNICORN Data System.
