All Values Available
No. American Indian or Alaskan Native, Asian or Asian American, Black or African American, Hawaiian or Other Pacific Islander, and/or White or Caucasian can be selected. Error message will appear if Declined or Unknown is selected.
Service Plan Element
Consent Date & Projected Start Date
Yes. When adding a new service to the IFSP the Consent Date for the new service is required. The Project Start date will populate 28 days from the Consent Date.
Provider Portal
My Eval Availability
Rescheduled Appointments
No. If an evaluation is canceled with one EI Referral Case and rescheduled with a different EI Referral Case, the calendar will display the most recently scheduled EI Referral Case details. My Client Cases for Evaluation list remains accurate.
Physical Address
If different than Mailing Address
No. When an EI Referral Case indicates that the Physical Address is different from the Mailing Address both addresses will display on the EI Referral Case details page from the Provider Portal.
